Reaching into a cabinet. Putting luggage in an overhead bin. Throwing a ball. Pulling on a shirt.
These and other everyday activities require us to lift our arms overhead without much thought – that is, until shoulder pain makes these movements difficult, disrupting our daily lives. In fact, pain when reaching overhead is one of the most common reasons patients seek evaluation for shoulder concerns.
Though occasional soreness after increased activity may not be unusual, recurring pain during overhead movement often indicates irritation within the structures that help the shoulder move smoothly and remain stable. Because the shoulder is one of the most mobile joints of the body, it relies heavily on its muscles, tendons and soft tissues working in harmony. When a component becomes irritated or injured, even routine movements can become uncomfortable.
One possible cause is a rotator cuff tear or irritation, which involves muscles and tendons that stabilize and move the shoulder joint. The rotator cuff plays an important role in lifting and rotating the arm, and repetitive activity, aging or overuse might at times result in inflammation or tendon degeneration. According to the American Academy of Orthopaedic Surgeons (AAOS), rotator cuff problems are a common source of shoulder pain and may initially present as discomfort with overhead movement or as pain at night. In some cases, patients notice symptoms gradually. What begins as mild discomfort during activity can progress to weakness, stiffness, or pain during everyday tasks.
Another potential source of overhead pain is shoulder impingement, which occurs when tissues within the shoulder become compressed during movement. This can create a pinching sensation, particularly when lifting the arm above shoulder height. Additional symptoms patients frequently describe include discomfort while lifting objects, pain when reaching behind the back and increased symptoms after repetitive activity. Because shoulder impingement often develops over time, patients sometimes assume it is simply part of aging or overuse and delay evaluation.
The shoulder can also become painful due to labral tears. The labrum serves as a cushion between bones and helps stabilize the joint. Injury can sometimes cause sensations of catching, weakness, instability, or pain during specific movements. The National Institutes of Health (NIH) notes that shoulder pain may result from a variety of causes, ranging from tendon irritation to instability to degenerative changes, underscoring the importance of an accurate diagnosis rather than self-treatment alone.

The encouraging news is that many shoulder conditions respond well to non-surgical treatment, including activity modification, physical therapy, injections, or other conservative approaches designed to restore function and reduce pain.
